Hurricane Sandy Relief Fund Donation Form

Hurricane Sandy Relief Fund for Members

On behalf of the Chapter Staff, Executive Committee and Board of Directors I wish you all well in the aftermath of Sandy. Our thoughts and prayers go out to our friends throughout the North East who have suffered major damage and hardship as a result of the storm earlier this week. We are aware of the devastation it has caused many of you and your practices.

The donation to NYPTA will go directly to our members who have been affected by Hurricane Sandy.  Since NYPTA is not a “501(c)3” these hurricane disaster relief donations will not be tax deductible.

To help, you can donate by filling out this form and paying with your credit card or send a check to:

ATTN: Hurricane Sandy Relief Fund
5 Palisades Drive, Suite 330
Albany, NY  12205

In the memo section of your check please indicate the check is for the Hurricane Sandy Relief Fund.

In Good Health

Matthew Hyland
NYPTA President
